Look at the screen-shot (From Railworks)

Whats to stop some idot from opening the door

They are usually locked & bolted.

All the back doors of trains were like that they were locked from the inside and bolted from the outside so you could not open them

There would also be a pair of external cover boards which should have been fitted.
