My good lady and I spent yesterday doing the Caernarvon - Portmadog and return trip on the Welsh Highland, but due to the rain there were few if any photographers track side.
It's always baffled me as to why photographers have to get so close to moving locomotives and trains.
This type of incident is fortunately extremely rare though.

This incident happened on the Talyllyn. It could just as easily happened in the street to someone on a mobile. People just don't concentrate when they have a device in their hand!
